To expedite the registration process, we now have two ways you can pre-register.  Click the Patient Registration button, print and fill out the form, then give it to our receptionist when you arrive.  Or, if you prefer, you may click Online Registration, fill out the form on your computer, and your information will be transmitted instantly.  Either way, however, you must print and sign the Financial Policy form and bring it with you when you come.  Upon arrival you will be asked to present your driver's license and insurance card, then your registration will be complete.
