Setting up an appointment

  • If our medical staff ordered your physical therapy, you will receive a phone call within 24 hours to set up your initial appointment.*
  • If you have a physical therapy order from an outside source, you can call us at 731-984-7098 to set up physical therapy at our clinic.
  • If you think you would benefit from physical therapy but you don’t have an order from a doctor, or if you are unsure if physical therapy would benefit you, give us a call anytime to discuss your complaints and let us help you decide your best options. Call us at 731-984-7098.

After you have scheduled your initial physical therapy visit, please fill out the patient forms, below, before arriving at the clinic.

*We will inform you of your individual health insurance coverage and benefits for physical therapy prior to setting up your appointment time; in addition, we will answer any questions related to your individual benefits.


Your first visit

  1. Dress casual with loose-fitting clothes. Make sure we can expose the affected part of your body.
  2. Fill out our patient forms online before your visit to expedite your session.
  3. Arrive 10-15 minutes early to review your completed forms.
  4. Your first appointment will include a history review, initial evaluation and treatment. This will last about an hour.  
  5. Expect mild soreness after your first visit. Soreness is a common side effect of treatment due to moving tight, limited structures and beginning to address other deficits, such as weakness, posture and impaired mobility.
